Services include: Adult and Pediatrics (Well-Child Exams, Back to School and Sports Physicals), Prenatal Care, Family Planning and Gynecology, Chronic Disease Management, Behavioral Health (Individual, Family and Marriage Counseling), Specialty Care Referral and DENTAL SERVICES.

After Hours Care
If you have an emergency, please call 911 or go to the nearest hospital.
If you have a medical question or need to contact your Provider after hours, please call (909) 630-7927. One of our Providers will call you back! Often times, your questions can be answered and a medical appointment scheduled for the next day. There are also 24 hour Nurse Advice Lines available to managed care members. Look for the number on the back of your insurance card.
Post Hospital or Emergency Room Visit
Did you know that if you schedule an appointment with your Provider within 2-3 days of leaving the hospital or ER you will most likely prevent being readmitted for the same problem. Seeing your Provider at the health center is a wonderful way to avoid another hospital visit.
